I just got the same bug trying to go back to Novigrad after Following The Thread. I finally got into Novigrad, here is what I did: I still had to turn in the quest The Price of Honor, so I did that. Then, I fast traveled to the bridge in Kaer Trolde. From there, I fast traveled to White Orchard Crossroads, and then was able to fast travel to Novigrad. I'm not sure what step fixed it, but my suspicion is not having turned in Price of Honor first..

Well I have just hit this bug after 125 hours of play time!
Thought I would come back and do some of the DLC quests. I just completed Skellige's Most Wanted then attempted to fast travel to Velen for the Wolf and the cat quest, however kept CTD'ing. Then tried to fast travel anywhere (Not just velen) and hit a CTD everywhere I tried to go.
I guess I am stuck in Skellige until this is resolved?
EDIT: So further troubleshooting has the DLC 'Where the wolf and cat play.' causing this for me. If I disable and re-load a save from previous, I can fast travel between Skellige and Velen. As soon as I enable it, CTD.
Is there a way I can submit a crash dump/bug report?

TPR: Fast travel to Ard Skelid (e.g. Kaer Trolde) and then to Novigrad. It worked for me this way.

This worked for me. I traveled first to Kaer Trolde, then to the main square in Novigrad.

Just got this, got past it by fast travelling to the main Skellige island and then fast travelling to White Orchard and then on to Novigrad.
Not sure if there are any other consequences yet.

I've not personally seen issues with TON, but the other three could be problematic.
I was able to use Rannvaig or most of the other Skellige Fast Travel locations as a first stop, then Arette is a 'working' arrival point in Novigrad/NML and convenient for an arrival into Novigrad. The most recent attempt (with the tail-end of 'Following the Thread' and the start of 'Skellige's Most Wanted' failed from both Fryesdal and Rannvaig to Arette, but I was able to travel from Fyresdal via Vizima to Arette.
Generally I don't use fast travel much, preferring to travel overland/water, and only fast travel when I have to. When only switching regions though I don't see the point of a long sea voyage merely to use a different fast travel point than the one I started next to though, so will use fast travel to displace if I cannot use my local one for some reason.

Happened when i had the 'bywaters murder' quest active. went back to a previous save completed that quest and was able to fast travel to and from Skellige. Now 5 levels on and a load of other quests im having the same problem again. trying to fast travel by boat or via White orchard ect is having no luck. basically anywhere in Skellige i try to travel from to anywhere that is not skellige causes a ctd

I have found a work around as I was encountering this bug, during 'Following the Thread' when I had to return to Novigrad, if you select another quest to track the bug doesn't happen. I selected 'Collect 'Em All' and was able to return without it crashing

I just discovered this bug this week. Can't get back to Novigrad from Skellige. Tried traveling by boat and deselecting Following The Thread as well (which is why I was going back).
Going to stop playing for a while and hope they patch this I guess..
